New report highlights patient and carer experiences of hospital discharge in Leicester and Leicestershire

Our latest report ‘Improving Hospital Discharge: What patients and carers told us’ shares feedback from 119 patients and carers about their hospital discharge experiences across Leicester and Leicestershire.

While many people praised the kindness and professionalism of NHS staff, the findings show ongoing challenges, including:

  • Communication gaps about discharge and aftercare
  • Delays receiving medication
  • Limited support and information for carers

People praised the compassion and professionalism of hospital staff, but many described confusion about discharge timings, aftercare arrangements and available support.

Improving hospital discharge requires strong partnership working across hospitals, community services and social care.”

The report highlights what is working well and sets out ten clear recommendations to help services improve how people transition safely from hospital to home.
